New Patient Registration
We are now accepting new patients. Previous growth has allowed us to have enough clinicians to allow sit and wait to happen, develop specialist asthma and COPD clinics and specialist minor surgery clinics.
Expansion of the nursing team, increased capacity to deal with the local explosion in the number of diabetics that we are caring for. We have brought in online consulting systems and extended access evening appointments for patients who are unable to get to the surgery due to work commitments. The expansion of the team has reduced the wait for routine appointments from one week to now consistently under 3 working days, and often even next day.
Opening the list will allow for more doctors and nurses to come into the practice, allowing for our GP's to take on leadership positions in the locality, to expand online consulting so that this will mean that we can streamline services to patients even more to improve responsiveness of the surgery to medical need. It will also allow for doctors to bring new specialisms into the clinic, hopefully reducing the need for patients to go to hospital.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. The questionnaire can be completed online using the link below.
Online "New Patient Health Questionnaire"
When you have completed the form, click the "Send" button to mail the form to us. Please note that we require both forms to complete your registration. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
